Acne breakouts and abscesses are caused by an interaction between hormones, skin oils, and bacteria that results in inflammation within the hair follicles. Both cysts and abscesses are pus-filled pockets, but abscesses are somewhat larger and deeper.
Sebaceous glands, which secrete an oily substance (sebum), lie in the dermis, the middle layer of skin. These glands are attached to the hair follicles. The sebum, along with dead skin cells, passes up from the sebaceous gland and hair follicle and out to the surface of the skin through the pores.
Acne results when a collection of dried sebum, dead skin cells, and bacteria clog the hair follicles, blocking the sebum from leaving through the pores or when the outflow of sebum injures the cells lining the sebum canals.
Inside blocked sebum-filled hair follicles bacteria of the type known as Propionibacterium acnes, which are normally present in the hair follicle grow and reproduce out of control. These bacteria break down the sebum into substances that irritate the skin. The immune system reacts with inflammation and the infection produces skin eruptions that are commonly known as acne pimples. If the infection worsens, an abscess may form, which may rupture into the skin, creating even more inflammation.
In treatments concerning more severe cases of acne, you'll need to find an aggressive cystic acne treatment. A common regimen used for treating cystic acne involves the use of isotretinoin and antibiotics. However, there are times when it may be necessary to use corticosteroid injections to help dissolve the nodule.
Types of Acne Irritations
Whiteheads: when a pore is completely blocked, trapping sebum (oil), bacteria, and dead skin cells, causing a white appearance on the surface. Whiteheads are normally quicker in life cycle than blackheads
Blackheads: Are when a pore is only partially blocked, allowing some of the trapped sebum (oil), bacteria, and dead skin cells to slowly drain to the surface. The black color is not caused by dirt; it is a reaction of the skin's own pigment, melanin, reacting with the oxygen in the air. A blackhead tends to be a stable structure, and can often take a long time to clear.
Papules: Papules are small, red, tender bumps with no head. Do not squeeze a papule, may cause exacerbate scarring.
Pustules: A pustule is similar to a whitehead, but is inflamed, and appears as a red circle with a white or yellow center. Pustules are your garden variety "zit".
Common Causes of Acne
Most dermatologists will tell you that the exact cause is unknown, probably because there is not any one cause that can be determined for everyone. It is common to say that acne is caused by the over activity of the sebaceous glands that secrete oily substances onto the skin.
There are many factors that can cause this development to take place: hormones, heredity and stress.
Mild Acne
People with mild (superficial) acne develop only a few non inflamed blackheads or a moderate number of small, mildly irritated pimples. Most acne occurs on the face but is also common on the shoulders, back, and upper chest.
Blackheads appear as tiny, dark dots at the center of a small swelling of normal-colored skin. Pimples are mildly uncomfortable and have a dark center surrounded by a small area of reddened skin
Mild acne usually does not leave scars. However, squeezing pimples or trying to open them in other ways increases inflammation and the depth of injury to the skin, making scarring more likely.
Severe Acne
Severe acne is rare, but can be disfiguring and, like all forms of acne, can have psychological effects on the sufferer.
The nodules and abscesses of severe acne often rupture and, after healing, typically leave scars. Scars may be tiny, deep holes (ice pick scars); wider pits of varying depth; or large, irregular indentations. Acne scars last a lifetime and, for some people, are cosmetically significant, and a source of psychological stress.
Types of Severe Acne
Acne Conglobata: This is the most severe form of acne vulgaris and is more common in males. It is characterized by numerous large lesions, which are sometimes interconnected, along with widespread blackheads. It can cause severe, irrevocable damage to the skin, and disfiguring scarring. It is found on the face, chest, back, buttocks, upper arms, and thighs. The age of onset for acne conglobata is usually between 18 and 30 years, and the condition can stay active for many years, and although acne conglobata is sometimes resistant to treatment, it can often be controlled through aggressive treatment over time.
Acne Fulminans: This is an abrupt onset of acne conglobata which normally afflicts young men. Symptoms of severe nodulocystic, often ulcerating acne are apparent. As with acne conglobata, extreme, disfiguring scarring is common. Acne fulminans is unique in that it also includes a fever and aching of the joints. Acne fulminans does not respond well to antibiotics.
Gram-Negative Folliculitis: This condition is a bacterial infection characterized by pustules and cysts, possibly occurring as a complication resulting from a long term antibiotic treatment of acne vulgaris. It is a rare condition, and we do not know if it is more common in males or females.
Pyoderma Faciale: This type of severe facial acne affects only females, usually between the ages of 20 to 40 years old, and is characterized by painful large nodules, pustules and sores which may leave scarring. It begins abruptly, and may occur on the skin of a woman who has never had acne before. It is confined to the face, and usually does not last longer than one year, but can wreak havoc in a very short time.
Treatments for Acne
General care of acne is very simple. Affected areas should be gently washed once or twice a day with a mild soap. Antibacterial or abrasive soaps, alcohol pads, and heavy frequent scrubbing provide no added benefit and may further irritate the skin. Cosmetics should be water-based; very greasy products can worsen acne. Although there are no restrictions on specific foods (for example, pizza or chocolate), a healthy, balanced diet should be followed.
Beyond these routine measures, acne treatment depends on the severity of the condition. Mild acne requires the simplest treatment, which poses the fewest risks of side effects. More severe acne or acne that does not respond to preliminary treatment requires additional treatment.

Treatment for moderate to severe acne
If the acne does not respond well to OTC treatments, or is visibly a moderate to severe form of acne vulgaris including nodules or cysts, lifestyle changes and medical treatment under the guidance of a doctor (most likely a dermatologist) is recommended. Doctor prescribed medications come in two basic forms, topical and oral. Treatment plans may include a combination of both types.
Oral Prescription Medications:
Oral acne medications are either antibiotic or retinoid in nature and common treatment methods in cases of moderate to severe acne including nodules or cysts. Oral antibiotics aim to control the acne by halting bacterial growth and reducing inflammation. Common antibiotics used to treat acne include: tretracycline, minocycline, doxycycline, and lymecycline. Oral antibiotics have stronger side-effects than their topical counterpart and may include reducing the effectiveness of birth control pills, as well as sensitivity to the sun for those who take tetracycline.
Even though they will reduce the amount of acne causing bacteria, oral antibiotics do not reduce the oil secretion (sebum) and abnormal cell behavior that causes pores to become plugged. Acne is becoming more and more resistant to antibiotics and will return if you stop using them.
To stop or slow the over-production of oil from the glands, your dermatologist may prescribe oral retinoids. A vitamin A derivative, oral retinoids such as isotretinoin (brand name Accutane) have shown themselves to be effective in treating 80 percent of moderate to severe acne cases. "The drug has a much longer effect than anti-bacterial treatments and will often cure acne for long. The treatment requires close medical supervision by a dermatologist because the drug has many known side effects (which can be severe)."
Your doctor will review the possible side-effects with you which include: dry skin, nosebleeds, and an initial flare up of acne within a month of taking the medicine.
The most severe side-effect, however, is for women who are or may become pregnant. According to the National Institue of Health's MedlinePlus Drug Encyclopedia: "Isotretinoin must not be used to treat women who are able to bear children unless other forms of treatment have been tried first and have failed. Isotretinoin must not be taken during pregnancy because it causes birth defects in humans. If you are able to bear children, it is very important that you read, understand, and follow the pregnancy warnings for isotretinoin."
Because of this danger to possible pregnancies, the National Institue of Arthritis and Musculoskeletal and Skin Diseases recommends that women take 2 forms of birth control beginning 1 month before treatment, lasting the entire dosage period and for one month after discontinuation of isotretinoin dosage. Iin most cases, isotretinoin may only have to be taken for 4-6 months.
About 25% of patients may relapse after one treatment. In those cases, a second treatment for another 4-6 months may be indicated to obtain desired results. It is often recommended that one lets a few months pass between the two treatments, because the condition can actually improve somewhat in the time after stopping the treatment and waiting a few months also give the body a chance to recover. Occasionally a third or even a fourth course is used, but the benefits are often less substantial.
Other Treatment Options:
In addition to the topical and oral treatments mentioned above, your doctor may employ one or more of the following:
- Hormonal Treatments: Since it is known that the male hormone, androgen, plays a role in the over-production of sebum, females may benefit from certain combinations of estrogen (female hormone) and anti-androgen medications. A woman with severe acne that worsens with her menstrual period may be helped by taking oral contraceptives. This treatment takes 2 to 4 months to produce results.
- Cortisone (steroid): Cortisone has been widely used in medical applications as an anti-inflammatory agent. In cases of severe acne, nodules or cysts, your doctor may inject cortisone directly into the acne lesion. This method works almost immediately by reducing the inflammation and decreasing the redness. This is an effective method in reducing the scars that left behind by severe acne, nodules or cysts.
- Phototherapy: Recent developments in the use of photo or light therapy, absent harmful UV rays, via a specific device has shown remarkable results for the treatment of acne. According to the BBC, doctors in London tested a red and blue light mixture on patients during 12 week study and found that it decreased visible spots by 76 percent.
- Surgery: In some cases, doctors will remove blackheads or whiteheads with a special comedone extractor. Doctors may also use a sharp instrument to drain the lesion of pus, oil and bacteria which relieves the pain, pressure and quickens the healing time.
- Cryotherapy: While rarely used, cryotherapy uses liquid nitrogen to "freeze" and break up or flake off the lesion.
Treatment of severe acne scars depends on their shape, depth, and location. Individual scars of any depth may be cut out and the skin sewn back together.
Doctors sometimes treat large, inflamed nodules or abscesses by injecting corticosteroids into them. Occasionally, a doctor cut open a nodule or abscesses to drain it.
